Welcome to the Parma School District Employee Portal 
This page was designed for you to find all the Human Resource information you need as an employee. If you click on the "Forms" page, you will find all of the employee forms and additional information. Throughout the year, if you need to make changes to your direct deposit, dependent information, tax claims (W4), address changes, etc. you can find it on the website. Simply print the form or fill it out on your computer and email/drop it off to the District Office.
 
**If you have an address change, you will need to fill out a new W-4 as well as new insurance forms.**
**If you have a name change, you will need to fill out a new W-4, I9 (with proof of identification in the form of a Driver's License, Social Secuirty Card, Passport or Birth Certificate) as well as new insurance forms.**
