Lions Club Vision Screenings will be Wednesday, September 20, 2023 starting at 8 am at Maxine Johnson Elementary, Room 13. All Kindergarten, 2nd, & 4th graders and any new students in the elementary school will be screened, but this is open to all students, teachers and staff that would like to participate.

Reminder – This Friday is the Immunization Clinic at the Elementary for ALL grade levels! Students MUST be Up-to-date on their immunizations by Monday, September 25^th to remain in school. See below for more information!

We are hosting an immunization clinic for all students Kinder - 12th Grade at the elementary on Friday, September 22nd. Please see the attached flyer for more information. Also attached is a letter from Southwest District Health, a Consent form and a copy of all of these forms in Spanish.

EXEMPTION FROM IMMUNIZATION Policy # 565
A student who supplies documentation to the district at the time of admission and before attendance of one (1) or both of the following conditions is not required to receive the required immunizations:
1. Life or Health Endangering Circumstances. A signed statement of a licensed physician that the student’s life or health would be endangered if any or all of the required immunizations are administered.
2. Religious or Other Objections. A signed statement of the parent, custodian, or legal guardian that must be either:
a. On a standard form of the Idaho Department of Health and Welfare or similar form provided by the school district; or
b. A signed statement that includes:
i. The name and date of birth of the student;
ii. A statement indicating that the student is exempt from immunization as provided by this policy and Idaho law for religious or other objections; and Immunization Requirements
iii. The signature of the parent, custodian, or legal guardian. The district will inform parents and guardians of applicable exemptions, with a citation to Idaho Code §39-4802, in its communications regarding immunization
